  2. 02
    Complete a CMDE

A Comprehensive Multi-Disciplinary Evaluation establishes medical need — including observation, medical exam, and assessments.

  3. 03
    Submit a referral

Complete the EIDBI referral form on the Minnesota Autism Resource Portal to receive provider information.

  4. 04
    Start services

We build an Individual Treatment Plan (ITP) with your family and begin services.
